Icodextrin is a water-soluble branched glucose polymer derived from starch, linked by α-(1→4) glycosidic bonds and less than 10% α-(1→6) glycosidic bonds, classifying it as a dextrin. Its weight-average molecular weight ranges from 13,000 to 19,000 Daltons, while its number-average molecular weight falls between 5,000 and 6,500 Daltons. As a new-generation osmotic agent alternative to glucose for peritoneal dialysis solutions, icodextrin features isotonicity, low glucose content and minimal metabolic byproducts.
Nevertheless, starch—the starting raw material for icodextrin manufacturing—contains abundant low-molecular-weight sugars, aluminum salts and magnesium salts. If dialysis fluid carries excessive aluminum salts, dialysis patients will absorb excess aluminum via peritoneal dialysis treatment. Long-term heavy administration of such dialysis fluid causes aluminum accumulation in the human body and induces toxic effects. Research indicates that once aluminum enters the human body, it primarily deposits in the brain and may trigger brain damage and severe memory loss, which are characteristic symptoms of Alzheimer’s disease. Additionally, residual magnesium salts and small molecular sugars in icodextrin drug substance directly compromise the quality of icodextrin peritoneal dialysis fluid, so magnesium salt control constitutes a critical research objective throughout the production development process.
